Two rules that keep it honest

  1. Every input must be observable without asking the customer how they feel. Surveys are a supplement, not a spine.
  2. Every band must have a defined action and an owner. A red account that produces awareness rather than intervention trains everyone to ignore red.

Signals that carry weight

Typical inputs, ranked by predictive value
SignalWhy it predicts risk
Onboarding milestone slippageThe strongest early indicator available
Issues open past SLAErodes trust faster than the issue itself
Falling customer response latencyDisengagement usually precedes the churn conversation
Consumption below planThe thing purchased is not being used
Checkpoint sentimentUseful only when captured consistently at the same moments

Common failure

Weighting the easiest signal to collect rather than the most predictive one, then defending the score when it disagrees with what the delivery team already knows. If the team's instinct and the score diverge repeatedly, the model is wrong — not the team.
